What is this study about?

This study is looking at a new way of providing healthcare in our local area, called 'Agir Sur Sa SantÉ' – which means 'Act on Your Health'. The main goal is to improve how we prevent and manage common long-term health conditions like type 2 diabetes, obesity, high blood pressure, and other heart-related problems. These conditions are often linked to our lifestyle, so this new plan focuses on helping people make positive changes like eating better, being more active, and stopping smoking.

Our healthcare leaders and a team of experts have developed this new plan to make sure everyone gets good quality care and can easily access the help they need. It's being introduced in local health centres and GP practices. This study will carefully examine how this new care plan is put into practice and whether it helps patients and healthcare staff. It will also look at whether people are happy with the care they receive and if it makes their care more joined-up and easy to access.

Ultimately, this project is important because it will give us valuable information about new models of care that combine prevention and management in local health settings. This could help shape how we deliver care for these conditions across the whole region, connecting local services with specialist care to provide continuous support.
